Services Offered: Cornerstone Hospice provides palliative care for patients (and families) who have advanced illnesses, or when life
expectancy is measured in months, not years. Care is provided wherever the patient lives, whether home, skilled nursing facility or assisted living facility. Special ancillary programs include the Tuck-In Program, available to hospice patients in Long Term Care and Assisted Living Facilities and the Pathways Program—Palliative care / support program for patients who have longer prognosis.
